Elena Van Gaver, ARNP, provides primary care for patients 16 and older. Elena has a special interest in chronic disease management such as diabetes care, preventive medicine, women’s health, gender-affirming care and addiction medicine. Elena strives to cultivate mutual connection, trust and understanding with her patients over time. Her approach is collaborative, working with each patient to address their health priorities and develop a personalized plan of care. Elena is a member of the American Association of Nurse Practitioners. She serves as a volunteer mentor with Young Women Empowered and as teacher in a nurse practitioner residency program.
